Abstract

Background The Hospice and Palliative Care Evaluation (HOPE) is a core documentation system and can be used for quality assurance and benchmarking. HOPE was repeatedly adapted to documentation needs following the yearly evaluations. This paper highlights development and changes of the hospice and palliative care scene in Germany over the last 5 years. Method During the evaluation periods in 2004 and 2009 participating services documented each 30 patients consecutively at the day of admission and at the end of treatment with the HOPE core documentation. This evaluation compared data from the palliative care units for the years 2004 and 2009. Results The evaluation showed significant differences for age (mean 68.7 vs. 67.3 years; p < 0.01) and a minor increase in the proportion of non-cancer patients (7.3 vs. 4.9 %; p < 0.01) compared to 2004. A significant higher proportion of patients had an advance directive (29.4 vs. 6.5 %; p < 0.01) and less patients were without a nursing support at home (47.1 vs. 37.7 %; p < 0.01). In both years the use of opioids WHO 3 and other medications for symptom control increased between admission and end of therapy, but antibiotics, antiarrhythmics / antihypertensives and diuretics were reduced. In both years high symptom intensities are estimated most often for weakness, assistance with the activities of daily living and loss of appetite. Best symptom relief is obtained for pain, nausea and vomiting and dyspnoea. Discussion The comparison with data from 5 years ago shows slow development of palliative care. Non-cancer patients still seem to find it difficult to access specialised services. However, public awareness on end-of-life care seems to increase, as indicated by more use of advanced directives in 2009.
